Porting a number form

When a customer wishes to prot a number to me, I as the reseller have them fill out the "Porting" form that VOIPo has and then I fax it or email it to VOIPo.

My question is this:

As the form has VOIPo's name in the letter, I DO NOT want my customers to see that. Can I change the areas that contain the words VOIPo and replace them with my company name?

Yes, you can change the company name. 99% of losing providers will accept it that way.
